Verizon Postpaid
Verizon Postpaid account # (on bill) ends in “-00001”. The Verizon Postpaid passcode is “call-in” passcode when you call customer service. It is usually the last 4-digits of your social security number (unless you specifically changed it to something else).

Viaero Wireless
To port out you will need to contact Viaero Wireless and request Account Number and PIN. You can also login on website with Username and Password to obtain your Account Number.
TPO Mobile
TPO Mobile account numbers and passcodes are only available by calling TPO Mobile customer service.
VTel
VTel does not offer Wireless services at this time, but plans to in the future. We will update this form with the porting instructions once we receive them.
Tracfone
Call TracFone and ask them to release your number for porting. Tracfone account # is NOT the phone number. It is the MEID or IMEI serial number of the phone (this can be found on the back of the phone itself – under the battery, so remove the battery!) Or, if your are using a BYOP SIM Card, it will be the last 15-digits of your SIM Card number. You can also view the account number in the My Account section of the TracFone website by clicking “Add (or Edit) Nickname”. – TracFone doesn’t use passcodes, use 0000 if the operator you are porting to requires a passcode.
Walmart Family Mobile
You need to call the customer service number, then request to release your phone number. Your account number is the serial number of the phone. For faster service, please dial 1-866-806-1840 and then dial this PIN: 623953
